Climbing Aerial Ladders

On June 21, 2007 firefighter Daniel Pudjak, Ladder 146, FDNY, climbing from the top of an aerial ladder to the roof of a four-story building lost his balance and fell 60 feet to his death. There were several contributing factors leading up to this fatality. [Read full article]


Propane Gas Explosions

A New York City man bleeding the remains of 20- pound barbecue propane gas tank in his yard on a windy day did not notice the gas drift into his open cellar doorway. The propane gas exploded, the blast collapsed the entire two story masonry wall of the house, blew out every window, seriously burned two occupants and drew a second- alarm response from the FDNY. [Read full article]
